Having trouble logging into the app? First try your username and password. If you do not have a username and password from your benefits portal you will need an Access Code. To retrieve the Access Code, log into your benefit portal and find the MyChoice Mobile App logo. For more help email [email protected].

From our quaran-team to you, our latest app release helps you more easily track COVID-19 with native functionality allowing you to easily log vaccination info, submit a vaccine exemption, or track weekly testing results. All vaccine information is self reported by the user and unverified by the application. We’ve added clarity to spending and savings accounts. Bugs have been squashed making Action Needed tasks accurate and bottom navigation behavior consistent. Enjoy!

Pinch and zoom right in on the details.

You asked for it! We did it. You can now register the MyChoice Mobile App with the same username and password you use to access your benefits portal. The traditional entry option using an access code is still available in case you really loved it or you don’t have an existing username and password. Additional background security measures alert members when new devices are registered.
