Description
The CareAdopt app is designed to connect an individual with a care program and a care provider. The individual typically has a medical or mental health condition that is chronic in nature (lasts more than 6 months). The app gives an individual a care program designed to assist the person in managing their condition in a way that helps the person live a happier and healthier life.
The care program delivered through the app comes with a daily list of tasks for the individual to perform including goals, tasks, assessments, symptom tracking, and vital tracking. The focus of the app is to enable individuals to engage in their treatment, focusing on self-management, and adoption of care.
The daily tasks given to the individual are centered on a type of condition. For example, there is a program designed for depression with assessments, vitals, and tasks centered on treating depression. There is another program centered on diabetes with assessments, vitals, and tasks meant for diabetes. Each individual engaged in the app receives a care program tailored to their needs.
The CareAdopt app used by the individual is connected to a complementary app used by a care provider. From the care provider app, the care provider is able to monitor the information and responses given by the individual; e.g., results of assessments, vitals, tasks, etc. to determine the effectiveness of the care program. The care provider can use the responses to determine if the care program needs to be modified and if the individual needs greater assistance.
The app includes direct secured messaging between the care provider and the individual. Through secured messaging, the app users can contact care providers, have message conversations with the care providers, and increase access to those assisting in their treatment and care.
The CareAdopt app is designed to increase the adoption of treatment for individuals in all care settings and assist care providers by increasing their assess to the individuals they are supporting.
