Georgia Memory Net is dedicated to the early diagnosis of Alzheimer's disease and related dementia. The first step to getting a diagnosis from Georgia Memory Net is an appointment with the patient's Primary Care Provider (PCP) to talk about memory concerns. If the PCP detects signs of memory loss, they can refer the patient to a Georgia Memory Net Memory Assessment Clinic for diagnosis via an in-person visit or telemedicine appointment. After diagnosis, Georgia Memory Net supports Primary Care Providers with a detailed care plan, and supports diagnosed Georgians and their families by connecting them to community services in their local area.
