Has your health care provider invited you to the Heart for Health app? With this app you can easily engage with your health, from the comfort of your home. Measurements that you submit are automatically forwarded to your care provider so they can monitor your health remotely.
The Heart for Health app offers the following
Safe log in
Each time you log in, we will send you a verification code via SMS, we do this to keep your account safe.
Sending measurements to your care provider
You can manually submit a measurement or use one of the connected devices to do so. Ask your care provider which devices are available to you. The measurements you submit are automatically forwarded to your care provider. You can also view your current and past measurements within the app.
Notifications and reminders
You will receive a message within the app when it’s time to perform a measurement.

A new and more extensive walkthrough is shown when a patient logs into the app for the first time. The walkthrough highlights the most important features and where to find them in the app.
The app allows for submitting manual blood glucose measurements as part of a new diabetes program.
Note: All new functionalities for App version 2 will only become available after transitioning to the new eHealth Portal V2.

By the implementation of a new functionality, you can now add measurements that were performed in the past. This can be easily done by adjusting the date and time. Furthermore there are some technical improvement done to increase the usability of the app.
