The myGM PHR app is designed to help you better manage your own health and care. myGM PHR has been designed for use across Greater Manchester and enables residents enrolled for home monitoring to add their symptoms to facilitate closer collaboration with their care teams.

myGM PHR is compatible with most smartphones.
