Upon launching the app, a button on the welcome screen will take you to either the Extensions preferences pane in Safari on Mac or the Settings app on iPad and iPhone. Click the checkbox next to the item called Skip Ad. Open youtube.com on Safari then tap "Always Allow for this website" and you're all set.
