Thanks to better and often complex treatments a still increasing number of cancer patients will survive their disease. After completion of treatment patients will receive so called survivorship care during their follow-up program. The focus of survivorship care lies on managing aftereffects of the treatment and early detection of a recurrence.
Survivors are confronted with several challenges: to recover from their cancer treatment and restore and maintain their health and overall well-being. The Survivor app supports patients during this process. The app includes information on recovering and remaining healthy after cancer treatment. It has interactive tools to keep track of questions to ask doctors and record voice answers. You can make pictures to illustrate both questions and answers.
The Survivor app contains some additional high-end features that are initially only available for survivors of testicular cancer, the most common type of cancer among young men.
There is a need to provide cancer survivors and their healthcare providers with a comprehensive survivorship care plan (SCP). The SCP includes a summary of the received treatment and a follow-up plan. This SCP assists the survivors and their caregivers to navigate the survivorship phase.
The Survivor app can read a QR code in which a personalized care plan is encoded. This QR code is generated by your specialist at the outpatient clinic. The SCP is tailored for the individual survivor of testicular cancer. The app provides you as a survivor with a comprehensive overview of your care plan which allows you to manage to-do’s and appointments with your caregivers and set reminders.
