Description

The Care Card was created for all the times that your loved one with Alzheimer's or dementia is receiving care, but you're not there. It is a digital snapshot of their needs, preferences, likes, and dislikes - designed to help any care provider better understand them as a person and how to make them more comfortable. Awesome tool for families and caregivers

This helps alleviate so many fears about family keeping in the loop and emergency officials informed should there be a problem. Definitely give it a try!
