Description

Empowering and supporting you along your healthcare journey

Handling your healthcare can be complicated… between medications, appointments, and activities, it’s easy to lose track of it all. Wouldn’t it be nice to have all your information in one place? Care4Today® Connect is designed to help empower you to take a more active role in your health, with helpful medication and appointment reminders and tracking capabilities for key health measures. Brought to you by Johnson & Johnson Health and Wellness Solutions, Inc., and created in consultation with patients and healthcare professionals.

This application can help you track your progress and provide support and motivation in between healthcare visits in the following ways:

• Delivers medication, medication refill, and appointment reminders that you set up to help you stay on track
• Records information you enter such as blood pressure, exercise, mood, etc. and tracks those reported measurements to help you see your trends
• Allows you to link external fitness apps like Apple Health to pull in your data around sleep, step count, and more
• Enables you to share graphs and trends on your self-reported data with your healthcare professional allowing for more productive conversations during healthcare visits
• Provides access to medications, appointments, and self-directed adherence data and organizes it all in one place

Care4Today® Connect is not intended to be relied upon as a primary method for scheduling when to take your medications. You are responsible for ensuring that you take your medication at the correct times and dosages. Always consult with your doctor about what medications to take, when to take your medications, and your health-related activities. Self-reported health measures are not intended to substitute reporting directly to your healthcare professional. You should make your healthcare professional aware of any changes in your health status.

FONNYJAY,
Latest update...
I recently updated my iPhone with the latest (13.1) update and a new issue has started.

* the app has created multiple entries of my medication
* when I go to mark my meds as ‘taken’ it’s not showing that so I get a notification to take it.

Also, would it be possible to be able to go back to update days you forgot to change? You can go back a day but would it be possible to make it a week?

Still enjoy using this app but just a few minor touches need to happen to make it awesome! 😊
magical mark,
Worst support ever.
They waited to have a solution to move your medications from the old app to this new one until two weeks before the old app is turned off. I have tried multiple times to migrate over. I emailed support and they stated "We are experiencing some technical difficulties with the migration too".

Then they said "We suggest that you manually input your medications into the new Connect app". and " The Mobile Health Manager app will be shut off at 9am ET tomorrow, August 31."

So much for customer service. I have 14 medications to transfer over. Why would I want to manually enter them so I can enjoy this poor level of customer service?
Mrs.Sturt,
2 Activities Missing
There should be an option to add an activity of being able to track menstrual cycles and maybe an activity of making a note. That way you can add a note about a certain day explaining your pain level or anything else necessary. It would make it easier to communicate with our doctors about specific incidents if we made a note about it THAT day.
StarCat33,
Issues after Apple ios update
After my iphone 7 updated to the current ios I was no longer able to login. It now says “data is in incorrect format”
almita33,
Care4ever
Really enjoy the features on this app it'll come in handy when I'm sick and I have to take medication. More than that, it reminds me of all of my doctors appointments.
EdSeal,
Please add Care4Family feature
I LOVED the look and feel of Care4Today and I will get used to the new app, but I would really appreciate it if you would put in the Care4Family feature. It could be a life saver!
Joker1,
Great app
I’ve used this app for a month now and it has definitely helped me keep track of my meds and appointments.
ccweems,
Only daily medication frequency
I need to take meds 3 times per day. It has a “custom” feature but it doesn’t allow any frequency more often than one per day. I expect that those considering using this app have serious requirements. It’s obvious that this app was never reviewed by a physician or pharmacist.
Barkel's Mom,
Understood but very disappointed
I know that with iOS 11 coming shortly that is why you discontinued the old app but you should have figured out a way to convert our YEARS OF DATA RECORDS over. It's valuable medical information we don't want to loose. ITS MECICAL.
Dry_Hole,
Slow and frustrating app
This is the most poorly developed application I have used in my iPhone.

It can take more than 30 minutes for the application to be ready to accept input. And I have a fast cable modem link.

I have used the new version since the old version was eliminated. The old version was much better than this “improved” version.

I have removed this app from my phone - I should have done this months ago.
