When my company first announced this program as a benefit I thought I need to research this company and see exactly what they do. I was very impressed that Forma handles so many companies. I had no idea this benefit was an option or even available. I was impressed with the company who told their employees they had to take a vacation away from the company. That made my day. If only my company would say that. I am so happy my company has researched a need for their employees and has offered such a generous benefit. Myself personally I had a hard time deciding what exactly I wanted or needed. My family has always came first so to have an option to purchase something I wanted or needed was thrilling. Yes I still bought what my family wanted but I did purchase some items for myself. Thank you for handling my company’s benefit. It is such a pleasure to look forward to know I will get reimbursed for items I want or need. Thank you again Rhonda Johnson

I get nothing but great feedback from my employees who are using this. Our change management strategy was how easy a mobile app is. When we tested this in our initial exploring around ease of use, communication etc we found this to be the best experience for our entire organization. Our company today has 18,000 employees about 6,000 are US employees! This is a wonderful expereince and when I sat down with our Forma team we uncovered that 75% of our population was using the mobile vs web to access forma. Keep up the great work team! We love Forma and your partnership.

We rolled out Forma last year 2022. Part of our due diligence was looking for a digital experience for our employees. Forma’s mobile app was far superior from any other one we looked at. So user friendly, great UI, and easy to use. We can not say enough great things. I look for utilization as a metric and right now we have 92% of our workforce using their app and we have 15,500 employees.

Loving this new look!
Love the new look of Forma’s app. It’s also very convenient to file claims for reimbursement too. The app would automatically suggest you the most suitable category for the claim to be filed under by the keywords you’ve entered in the description (so smart!). Also, the new changes to the Forma Store is awesome. Great variety of product selection too! All in all, 10/10. 🙌🏻

Beautifully Accessible Perks
My balance for employer funds is so easy to access when I am on the go and needing to double-check what I have left. I can even easily file a claim directly in the app so it is not forgotten or put off until later. The color graphics make for an eye-catching experience. Love it!

Annoying claim denying tards
They reject every 3rd claim or so for getting back your own hard earned money. Claiming insufficient info, even though all documents, bills, credit card transaction info was already provided. They even once denied ever receiving the claim then again denied it when submitted again for being a dupe, then denied it a third time after not responding within the claim period because now it’s out of the allowed time range. I hope as penance for this horrible service that all of their family is required to use it and their claims get denied at 3x the normal rate.

The easiest way to submit claims!
Super convenient to file a claim from my phone and simply select a picture of the receipt straight from my camera roll. Also like getting push notifications about claim status and account activity. Overall great and easy to use!

This app is so easy to use. They are fast at processing a request and never give you a difficult time or hold up a claim. I love that we can get reimbursed for things that we actually need and use, not only narrowed down to one category like and hsa.

Easy way to submit claims!
I love using Forma to submit claims for my purchases that are covered by company-subsidized funds (i.e wellness).
The process is easy and painless - just upload a receipt and enter a few details (takes less than 5 min), and someone will generally approve it within a day or two.

Impressive App and Easy Process
I am lucky to work for an amazing company like Microsoft who provides the benefit of Forma! Being able to do everything real time in the app is a huge plus. This app makes the process clean, easy, and enjoyable!
