Opioids by Chris Marcellino MD and Norris Vivatrat MD is an opioid analgesic dosage conversion calculator for use by trained medical professionals. The application supports conversion to and from opioids commonly prescribed and used in U.S. hospitals orally, parenterally, or transdermally, including morphine, codeine, hydromorphone, hydrocodone, fentanyl, methadone, oxycodone and oxymorphone. The application is intended to help and instruct in the process of conversion of opioid regimens.

Opioid equipotency data is taken from CAPC conversion values except where noted (see the reference tab in the app.) CAPC values are as published in the Center to Advance Palliative Care Prescribing Opioids: A Reference Guide curriculum, which are adapted from McPherson ML. Demystifying Opioid Conversion Calculations: A Guide for Effective Dosing, 2nd ed. American Society of Health-System Pharmacists, Bethesda, Maryland, 2018. EPEC values are as published in EPEC-O Participants Handbook, 2005.

Use traditional conversion (1:2) for fentanyl based on clinical experience and increased safety (regardless of incomplete cross tolerance) when transitioning to oral opioids from transdermal fentanyl.

Pretty good opioid calculator
When I upgraded my iPhone to iOS 8, I lost the ability to use my original opioid calculator app, the opioid. This is a fairly suitable replacement, and it works well with good stability on my iPhone 5S running iOS 8. The only suggestion I would have is that it is cumbersome to enter in numerical quantities for milligrams received using "rollers" instead of a keypad.

I use it everyday working in pain management! The only improvements you could make is add Nucynta and a two a day dose for Fentanyl Patch!!

Great, accurate, fast, useful, cool logo
Used this almost daily on the Medicine wards. Far superior to the one in MedCalc. Fast, accurate, and useful. It's clear that the authors spent a lot of thought and had expertise in this area.

In particular, the links to the Reference material give you all the details you need to have confidence in dosing opiates.

I was particularly impressed by the Methadone dosing, which is not easy to do, and not done accurately in MedCalc. Check out the "Rule of 15's" in the reference material

Very cumbersome, and needs better user interface. Needs update.

Does not include Tapentadol or many other opioids. Only dosing options are q6h, q8h and w24h.

Easy to use conversion calculator - based on equianalgesic doses many docs use
This calculator is easy to use, and references a commonly used equianalgesic dosing table.
I recommend checking with your health-system to see whether this is the ratio card your institution uses.
*it includes ratio options taught through EPEC - which is what major academic institutions use, including Institute for Palliative Care at San Diego Hospice.
I disagree with the previous commentator suggesting it uses dangerous conversions.

Allows conversion for patients on multiple meds
Allows for the entry of multiple opioids that can merge into a single conversion recommendation. Well annotated. Other apps I have used required the use of scratch paper to write down intermediate results. Not this one! Thanks for making conversions for my patients faster and safer.
